Team Volunteer Time Sheet FORM

To be filled out by March 31.

PLEASE NOTE: FIRST 3 ASSISTANT COCHES WILL BE THE COACHES WHO RECEIVE VOLUNTEER HOURS.
Please list all staff (coaches, student coaches, manager) for your team. Please confer with the head coach to make final decisions. Please remember that anyone participating with your team must be registered individually and must be rostered to your team. This means for practices and games, on the ice or the bench. If someone is not on the roster, they may not participate with your team.

Injury Report Form

PER POLICY AND PROCEDURES:
Any skater involved in an injury requiring physician’s attention will not be permitted to return to the ice (practices or games) without the treating physician’s written release. The release should be given to the head coach with a copy to VP on Ice and respective level directors to avoid further injury. All on ice injuries must be reported within one week to the appropriate level director.

Team managers are responsible to fill out the injury form from the website and submit to the VP Off Ice,VP On Ice and the respective level director .
